How to refer your patients to us

Individual occupational therapy follow-up

  • Pain or functional limitation of the musculoskeletal system affecting daily or work activities
  • After an injury or surgical procedure (shoulder, spine, knee, etc.)
  • Need to adapt an activity, environment or equipment

Regaining the movements that matter.

The occupational therapist works from the patient’s real-life activities, at home and at work, adapting movement, environment and equipment.

Documents to send:

Rachis Programme®

  • Persistent or recurring lower back, mid-back or neck pain
  • Impact on daily or professional activities
  • Patient ready to commit to an active approach

Outpatient interdisciplinary functional rehabilitation dedicated to back issues. The programme combines occupational therapy and physiotherapy, in individual and group sessions.

Documents to send:

Evaluation of Occupational Performance (EPO)

  • Question about work capacity or return to work after leave
  • Need to objectively document capacities (insurer, disability insurance (AI), employer)
  • Guidance on management when the clinical picture is complex

An objective measurement of the patient’s physical capacities, through a series of functional tests representative of the demands of their daily life and job.

Documents to send:

Pre- and post-operative programme (P.O.P.O.)

  • Planned musculoskeletal surgery (spine, shoulder, hip, knee, etc.)
  • Post-operative recovery requiring coordinated rehabilitation
  • Concerns around resuming activity or returning to work

Preparing before surgery, recovering afterwards. This pathway structures the weeks before and after musculoskeletal surgery, in coordination with the surgeon.

Conservative management of traumatic and degenerative musculoskeletal conditions.
